Who can take part

  • You are 18 years or older.
  • You have one of these conditions: diabetes with A1C of 7.5 or higher, heart disease treated with a stent, coronary artery bypass surgery in the last year, or chronic kidney disease (stage 2–4).
  • If you also have heart failure, you need to have been hospitalized for it within the past week.
  • You are willing to sign a consent form.
