This study tests whether extra teaching from a nurse helps people with heart failure take better care of themselves and stay healthier after leaving the hospital. If you join, a nurse will work with you to help you understand your condition and manage it at home.

- You are 18 or older
- You are in the hospital being treated for heart failure
- You are between NYHA class II and IV (moderate to severe symptoms)
- You have a follow-up heart appointment scheduled about a month after leaving the hospital
- You do not have severe memory or thinking problems
