Who can take part
- You are at least 18 years old
- You were recently discharged from the hospital with a heart failure diagnosis
- Your heart failure symptoms at discharge were at least moderate (NYHA class II, III, or IV)
- Your first heart doctor follow-up appointment is scheduled 15 to 45 days after leaving the hospital
- You have not been in a heart failure clinic program before
- You have not had a heart attack or acute heart event in the last 3 months
