- You are over 18 years old
- You’ve been seen by a cardiologist at an Inova clinic or hospital
- Your heart pumping test shows left ventricular ejection fraction (LVEF) is 40% or less
- You have not chosen to opt out of research that uses electronic health records (EHR)
- You are not on hospice care
- You are not already on the maximum doses of all four standard heart-failure medication groups
