- You are coming to your first appointment at the UCLH Facial Pain Service or Oral Surgery Clinic for facial or oral pain
- You have been referred by a primary care doctor for this pain
- You can understand English well enough to complete a questionnaire
- You have enough time before your appointment to read the information, give consent, and fill out the questionnaire
