Who can take part
- Be at least 18 years old
- Arrive at the emergency department with severe low oxygen levels (fast breathing, needing high oxygen, or already on breathing support)
- Need non-invasive breathing support (mask or high-flow oxygen, not a breathing tube)
- Be enrolled and randomized within 2 hours of identifying the need for breathing support and within 6 hours of arriving at the ED
- Not have an urgent need for intubation (breathing tube) right away
- Not have a heart rhythm emergency, stopped breathing, facial anatomy that prevents mask use, or a tracheostomy
