Who can take part
- You are 18–70 years old and have lived in Paraguay for at least 12 months
- You speak Spanish well enough to complete study forms and follow instructions
- You have not been hospitalized for diabetes or weight-related complications in the past 3 months
- Your kidneys work adequately (eGFR ≥45 mL/min/1.73m²)
- You have either type 2 diabetes (HbA1c 7.0–9.5%) on stable single or dual therapy, or obesity (BMI ≥30) without diabetes and a failed diet attempt in the past year
- You have not used GLP-1 drugs (like Ozempic, Victoza) or similar medications in the past 3 months, and have never used insulin
