- Age 18–80 with good overall health (able to do light activities)
- Rectal cancer confirmed by biopsy, with specific genetic markers (pMMR or MSS)
- Cancer at stage II or III, located within 12 cm of the anus, not previously treated with radiation, chemotherapy, or immunotherapy
- No immune system diseases, serious heart/lung/liver/kidney problems, or active infections
- Blood work showing adequate white blood cells, platelets, hemoglobin, and normal liver/kidney function within 2 weeks of starting
- Willing to use contraception (if able to become pregnant or sexually active) and provide tissue/blood samples for research
