Who can take part
- Spinal cord injury at the neck level (C1–C4), with significant paralysis in all four limbs
- Medically cleared for surgery and able to undergo brain imaging (MRI) before implant
- Able to communicate reliably by speech or other means, and mentally capable of informed consent
- Vision and hearing largely intact
- Live within reasonable distance of UMC Utrecht hospital in the Netherlands
- No progressive/degenerative spinal cord condition, active infections, severe swallowing problems, or certain implanted medical devices incompatible with the trial
