Who can take part
- You are 18–65 years old
- You have been diagnosed with heart failure (moderate severity, NYHA class II or III) for at least 6 months
- Your heart's pumping strength (ejection fraction) is 40% or lower, confirmed by heart ultrasound
- You own a mobile phone and can use it to receive messages and communicate
- You speak Amharic, Oromo, or English fluently
- You are able to do mild-to-moderate physical activity without medical contraindication
