- You are between 18 and 75 years old
- You have advanced heart failure with severe heart function problems (like an ejection fraction of 35% or less, or high BNP levels)

- You have been hospitalized for heart failure in the last 2 years
- You are on the best possible heart failure medications for more than 30 days
