This study tests whether taking a vitamin B3 supplement before and after heart surgery can reduce the chance of kidney damage. It is for people having certain heart surgeries with a heart-lung machine and who are at higher risk for kidney problems.

Who can take part
- You are 18 years or older.
- You are scheduled for heart surgery (bypass and/or valve) using a heart-lung machine at Beth Israel Deaconess Medical Center.
- Your doctor has calculated a kidney risk score of 6 or higher, meaning you have an increased chance of kidney issues after surgery.
- You have not had kidney injury in the week before surgery, such as a sudden rise in creatinine or need for dialysis.
- You do not have advanced chronic kidney disease (eGFR below 15 or already on dialysis).
- You have not taken nicotinamide (a form of vitamin B3) supplements in the last 30 days.
