If you come to the emergency department with very high blood pressure, this study offers a team-based approach to help manage it and connect you with ongoing care, instead of just treating the emergency and sending you home.

Who can take part
- You are between 18 and 85 years old.
- You had at least two blood pressure readings in the emergency room that were very high (systolic 180 or higher or diastolic 110 or higher).
- Your blood pressure care has been irregular, such as having had emergency visits for high blood pressure or not having a regular doctor.
- You are able to take part in English.
- You are expected to go home, not be admitted to the hospital.
- You do not have certain other conditions like late-stage kidney disease, severe heart failure, or a transplant, and you are not pregnant or planning pregnancy in the next year.
