This trial tests whether pyridostigmine helps people with postural tachycardia syndrome (POTS), which causes heart rate to rise when you stand up and can lead to unpleasant symptoms. You would be screened based on your tilt-table heart-rate response and your standing-related symptoms.

Who can take part

  • You have been diagnosed with POTS using a tilt test (heart rate rises by at least 30 beats per minute within 5 minutes of standing/tilting)
  • Your POTS symptoms happen when you stand up, such as lightheadedness, weakness, blurred vision, nausea, palpitations, or brain fog
  • You are not pregnant and not breastfeeding
  • You do not have thyroid problems (not uncontrolled hypothyroidism or hyperthyroidism)
  • You have not had recent pyridostigmine treatment for POTS (none in the past month)
  • You are able to participate safely without major other organ-system problems or heart artery disease, and you’re not on medicines that could interfere with the testing
