This study looks at how well the PACE/PACENET care program works for older adults over time. If you join, you’ll provide consent for a check-in about your outcomes about 3 months after the acute (most intense) phase of care ends, with follow-up around 6 months.

- Be age 65 or older
- Join or be participating in the SUSTAIN Clinical Program
- If you’re asked for the 6-month part, you agree to talk and consent
