- You are 21 years old or older
- Your vision has been getting worse over time, and glasses or contacts no longer work well enough for daily life
- Your central cornea (the middle part) must be clear
- Your cornea must be at least 450 microns thick at the planned incision spot
- A corneal transplant must be your only remaining option to improve vision
