Bone marrow transplant with lighter chemo for non-cancer illnesses

Severe Sickle Cell DiseaseBone Marrow Failure SyndromesMetabolic DisordersImmunologic DisordersHemoglobinopathiesNon-malignant Disorders

Part of Blood & lymphatic, Genetic & congenital, Hormones & metabolism, Immune system & allergy clinical trials.

This early-phase study tests a safer “lighter” conditioning chemo plan before a bone marrow transplant for people with non-cancer blood and immune disorders. It aims to reduce transplant risk while using donor marrow that does not match perfectly in the HLA type.

Who can take part

  • You need a bone marrow transplant for a non-cancer problem (like bone marrow failure, immune problems, metabolic disorders, or blood disorders).
  • If you have sickle cell disease, you must have severe symptoms (such as stroke/TCD issues, repeat lung episodes, kidney problems, frequent painful crises, ongoing transfusions, or repeated bone/joint injury).
  • If you have sickle cell disease, your hemoglobin S level must be under 30% within 30 days before starting the study drug alemtuzumab.
  • You must be 20 years old or younger at enrollment, and your heart and lung tests must be strong enough (echocardiogram and oxygen/lung function limits).
  • Your liver and kidney blood tests must be within allowed ranges, and you must not have known cirrhosis, bridging liver scarring, or active hepatitis.
  • You must not have an HLA-matched sibling donor available, and you must not have had a prior stem cell transplant.

NCT00920972Recruiting· Phase 1/Phase 2
Bone marrow transplant prep for children with non-cancer blood disorders

This early-stage study tests a specific chemotherapy “conditioning” regimen to prepare the body for a bone marrow transplant in children with certain non-cancer blood or immune conditions. It may help the transplant work better, but it also carries risks from the chemotherapy and transplant process.

NCT04099966Recruiting· Phase 2
Stem cell transplant to treat high-risk blood cancers

This trial tests a specialized donor stem cell transplant to treat high-risk problems in the blood and bone marrow, including some cancers and non-cancers. It may help by using donor immune cells designed to better control the disease after transplant.

Valhalla, New York
NCT05736419Recruiting· Phase 2
Immune-suppressing treatment before stem cell transplant for blood disorders

This Phase 2 study tests an immune-suppressing treatment for people with sickle cell disease or beta-thalassemia who are preparing for a stem cell transplant from a donor. It aims to help the transplant work better by changing the immune response.

Basking Ridge, New Jersey
NCT03653338Recruiting· Phase 1/Phase 2
Bone marrow transplant using donor blood to treat sickle cell

This trial tests a specific type of bone marrow transplant for people with sickle cell disease (or certain other blood disorders) who have had serious complications despite standard care. It aims to see if removing the patient’s immune cells and using a matched donor’s cells can improve outcomes and reduce complications.

NCT07493538Recruiting· Phase 2
Blood stem cell transplant with lower-intensity chemo for blood cancers

This trial tests a stem cell transplant using milder chemotherapy (treosulfan and fludarabine) followed by a drug called cyclophosphamide to prevent graft-versus-host disease. It aims to make transplant safer for people with blood diseases who cannot handle standard high-dose chemotherapy.

Minneapolis, Minnesota
NCT01962415Recruiting· Phase 2
Reduced-intensity stem cell transplant for non-cancer disorders

This trial tests whether a gentler (reduced-intensity) conditioning treatment before a stem cell transplant works for non-cancer diseases. It uses specific donor stem cell matches and checks heart, lung, liver, and kidney function to help lower transplant stress while still aiming for benefit.
