  • Be 18 years or older
  • Have symptoms that fit heart failure and at least one supporting heart-failure test result
  • Have heart failure type defined by heart pumping strength: preserved (EF 50% or higher) or reduced (EF under 50%)
  • Have not been on dialysis for kidney failure (end-stage renal disease)
