Test an ECMO approach without blood thinners for lung failure

Extracorporeal Membrane Oxygenation Complication

This trial studies whether people on a breathing machine called VV-ECMO can be managed without routine blood thinners, to reduce bleeding risk. It is meant for adults with severe lung failure (ARDS) and checks that it is safe for you to avoid anticoagulation.

Who can take part

  • Be an adult (over 18) and have ARDS, a severe lung inflammation/oxygen failure
  • Be receiving VV-ECMO (a lung-only support machine that filters the blood through a breathing circuit)
  • Not have a reason you cannot take blood thinners (example: heparin-induced low platelets, active bleeding, or surgery that prevents blood-thinner use)
  • Not currently need full-dose blood-thinning treatment for a clot (like pulmonary embolism or deep vein thrombosis)
  • Not be on very low ECMO blood flow (under 2 liters per minute)
