This Phase 2 trial tests whether milrinone, a heart-acting medicine, can improve brain blood flow to help prevent delayed brain ischemia after a severe subarachnoid hemorrhage. It may help reduce later brain damage in people who are too ill or sedated for a normal neurologic exam early on.

Who can take part

  • Be an adult (18 or older) with a severe subarachnoid hemorrhage (bleeding around the brain)
  • Be in a deep coma or needing sedation around day 3, so a neurologic exam can’t be done normally (Glasgow Coma Score 8 or less)
  • Have no major prior disability before this illness (mRS 0–2)
  • Have a family member or trusted person able to sign consent after being fully informed
  • Not have serious unstable heart or circulation problems at the time of randomization (for example heart failure needing strong heart-boosting drugs, severe low blood pressure, or septic shock)
  • Not have conditions that block safe treatment or testing (for example allergy to milrinone, severe kidney problems, low potassium, or an MRI contraindication)
