  • You have a history of lazy eye, or risk factors that can lead to lazy eye
  • Your eye problem involves reduced vision in one eye and/or eye misalignment
  • You do not have a past or current neurologic (brain or nerve) disorder
  • You may be asked about your medical and development history to confirm eligibility
