Who can take part

  • You are an adult (over 18 years old).
  • You are already on a breathing machine (intubated and on mechanical ventilation) for more than 24 hours.
  • Your lung oxygen levels are low despite breathing support (meeting the trial’s oxygen ratio threshold) and you need certain ventilator settings (FiO2 at least 0.30 and PEEP at least 5).
  • Your acute lung failure is caused by a problem in the lungs (a pulmonary cause).
  • There are no major safety issues that prevent using higher PEEP, such as very severe head or chest trauma.
  • You can safely lie flat (supine position), and blood pressure is stable enough that your team does not consider you hemodynamically unstable.
