This trial tests whether adding an “alkali” treatment (to raise low blood bicarbonate) can improve blood vessel and kidney graft function in people whose kidney transplant is about 1 year old. It may help if you have lower-than-normal bicarbonate levels while on stable transplant and blood pressure medicines.

Who can take part

  • You are age 18 to 80 and can give informed consent
  • You had a kidney transplant about 1 year before starting the study
  • Your blood bicarbonate is 16 to 24 on two blood tests at least 1 day apart
  • Your kidney function is fairly preserved (eGFR at least 45)
  • Your blood pressure is under 130/80 before randomization, and your medication plan is stable
  • You are on a stable transplant medicine plan: tacrolimus, mycophenolate mofetil, and prednisone
