- Be age 45 or older
- Have heart failure that is mild to moderate (you can do minimal activity without discomfort)
- Have been hospitalized for heart failure within the past 12 months
- Be comfortable using English and pass a brief memory/thinking screen
- Be able to walk on your own without a cane or walker
- Be able to use a smartphone and body sensors (no severe vision or touch problems)
