- You were diagnosed with premature ovarian insufficiency before age 38
- Your chromosome test is normal for 46,XX (and you do not have an FRM1 premutation)
- You have at least one sign your ovarian reserve is lower than expected for your age (like higher FSH, low AMH, or low AFC)
- Or, you previously had fertility treatment that did not respond well (few follicles or few eggs retrieved despite high-dose hormones)
- You have not had ovarian surgery or prior chemotherapy or radiation
- You do not have endometriosis, known autoimmune conditions, or certain metabolic diseases
