This Phase 3 study tests whether immunotherapy or targeted medicine, with or without a one-time focused brain radiation procedure, can control brain metastases from melanoma or non-small cell lung cancer. It mainly enrolls people who have no (or minimal) brain symptoms and are newly diagnosed in the brain (except for certain recent brain surgery).

Who can take part
- You are at least 18 years old, and your general functioning is moderate to good (Karnofsky 60 or higher).
- You have melanoma or non-small cell lung cancer that has spread to the brain, confirmed by a pathology report.
- Your brain metastases are new and have not been treated before (except surgery for diagnosis).
- Your brain lesions fit the size/number limits: up to 1–10 spots, with at least one at least 5 mm; and they must not be too large overall.
- Your brain symptoms are absent or minimal (“oligo-symptomatic”); you should not need more than a stable low dose of steroid medicine for more than a week.
- You must be able to take study treatment safely: life expectancy over 12 weeks, no prior whole-brain or prior focused brain radiation, and (for this study) no prior drug treatment specifically for brain metastases.
