Who can take part
- You are over 18 and have a kidney transplant
- You have a kidney infection with fever (at least 38°C) plus urine signs of infection, and your urine culture grows bacteria that match the antibiotic plan
- There is no other confirmed or suspected fever-causing infection besides the urine infection
- Your early response to the first antibiotic is improving within 48–60 hours (fever goes below 38°C and urine symptoms improve)
- Imaging (scan) before treatment shows no blockage and no abscess
- You can sign consent and follow the study rules
