- You are older than 18 years
- Your facial pain is diagnosed as classic trigeminal neuralgia
- Your pain has not been controlled well enough with the strongest (maximum) medicine you can tolerate
- Your pain level is severe (BNI score IV or V)
- Your trigeminal neuralgia is not caused by another condition like a tumor, abnormal blood vessel, or multiple sclerosis
