This trial tests whether giving serplulimab (an immune medicine) plus CAPEOX chemotherapy and celecoxib before surgery can shrink locally advanced rectal cancer and improve outcomes. Who can take part

  • You’re between 18 and 75 years old, and you can sign consent
  • Your rectal cancer is adenocarcinoma (documented by biopsy/pathology), and you haven’t had chemotherapy for rectal cancer before
  • Your tumor is within 10 cm of the anus, and MRI shows specific features (including a negative MRF) and the cancer stage fits cT2-4 and N0-2 without distant spread
  • Your health status is good enough (ECOG 0–1) and the trial team can test tumor tissue for MSI/MMR
  • You can participate based on virus/liver blood tests (for example, no hepatitis B or—if present—well-controlled levels; and no combined hepatitis B and C)
  • You have no major “reasons to stop,” like prior pelvic radiation, inflammatory bowel disease, active HIV/AIDS, recent major surgery, or prior immune checkpoint antibody treatment
