- You’re age 65 or older.
- You have stable angina from blocked heart arteries and a plan to start heart medicines.
- You have at least two other long-term conditions.
- You likely had testing showing significant heart artery narrowing (blood-flow or imaging results).
- You are not currently taking BOTH a beta-blocker and a calcium channel blocker.
- You can join follow-up visits and give informed consent (you may need to speak English or Spanish).
