Who can take part

  • You are age 45 or older
  • Your heart failure is due to ischemic heart disease (blocked/narrowed heart arteries), based on your doctor’s assessment
  • Your heart failure symptoms are New York Heart Association (NYHA) class II, III, or IV
  • Your heart’s pumping strength (LVEF) is 45% or less within the past year
  • You are on your usual heart failure medicines at the right doses for you (optimized treatment)
  • If you are currently in the hospital, you must be stable and likely to be discharged within 72 hours
