This trial tests whether adding a PSMA-targeted radioactive medicine (Lu-PSMA) to focused stereotactic radiation to a few prostate cancer spots works better than radiation alone. It may help people with limited, asymptomatic spread of prostate cancer by improving cancer control while carefully monitoring side effects.

Who can take part

  • You have prostate cancer with 1 to 3 symptom-free metastases (spread spots) that are small (each ≤5 cm or the total volume <250 cm³).
  • Your PSMA-PET/CT scan is positive, and the spots on that scan match what was seen on your baseline CT/MRI scan.
  • You must have already had treatment to the main prostate tumor (surgery and/or radiation), and prior “salvage” radiation to the prostate area or pelvis is allowed.
  • Your PSMA-PET/CT scan is strong enough (SUV max at least 3) and it was done within the last 3 months.
  • Your PSA and testosterone are in allowed ranges, and PSA is rising fast enough (PSA doubling time under 15 months).
  • You must be 18+ and well enough to participate (low limits on blood counts and liver/kidney tests, and overall health status ≤2), with at least 6 months of expected survival.
