This trial tests a new approach for patients with advanced non-small cell lung cancer that has a specific EGFR mutation (Ex19del or L858R) and has started to grow again after at least six months on osimertinib. It combines two types of focused radiation with continued osimertinib to treat only the progressing spots while keeping the rest of the disease under control.

Who can take part
- You must be 18 or older and able to give informed consent
- You must have advanced non-small cell lung cancer with a common EGFR mutation (Ex19del or L858R)
- You must have been on osimertinib as your first treatment for advanced disease and had stable disease for at least 6 months before the cancer started to grow again
- You must have 1 to 5 growing spots outside the brain that can be treated with focused radiation (stereotactic body radiation therapy or biologically guided radiation therapy)
- You must have at least one growing spot in the lung or bone that is 2 cm or larger for the biologically guided radiation type
- You must not have had any other chemotherapy, radiation, or immune therapy within 21 days before starting the study (except brain radiation or osimertinib)
