  • You are 45 years old or older
  • You have at least one risk factor for stroke, such as high blood pressure, diabetes, or heart disease
  • You have a history of heart disease, like a previous heart attack, angina, or a stent in a leg artery
  • You can speak and read English well enough to follow instructions
  • You live on your own or with help from family, not in a nursing home
  • You are willing to join the study and give permission
