Who can take part

  • You are 18 or older.
  • You have a HeartMate 3™ LVAD and it is your first one.
  • You can walk on your own (with or without a walker or cane).
  • You own a smartphone with internet access.
  • You will be discharged home with your LVAD or living at home within 45 days of discharge.
