  • You must have either type 2 diabetes or chronic heart failure.
  • You also need at least two other long-term health conditions (like high blood pressure, kidney problems, or arthritis).
  • You should have visited your GP practice or been referred to the hospital many times in the past two years.
  • You cannot take part if you have a terminal illness, severe mental health issues (like psychosis or dementia), or cognitive impairment.
