  • You must be 18 years or older.
  • You have chronic heart failure (with symptoms like shortness of breath, tiredness, or swelling), and your doctor has confirmed a specific type based on your ejection fraction (a measure of how well your heart pumps).
  • You are not pregnant, breastfeeding, or planning to become pregnant in the next year.
