This trial tests if virtual reality (VR) cognitive training before surgery can help prevent postoperative delirium (sudden confusion) in older adults with cerebral small vessel disease (a condition affecting small blood vessels in the brain). If you're 65 or older, have this brain condition, and are having non-cardiac surgery, you may be eligible.

Who can take part

  • You are 65 years or older
  • You have been diagnosed with cerebral small vessel disease (a condition of small brain blood vessels) by MRI
  • You are scheduled for non-cardiac, non-brain surgery under general anesthesia that is expected to last more than 2 hours
  • You have an ASA physical status of I, II, or III (a rating of your overall health before surgery)
  • You have not taken any memory- or thinking-enhancing medications in the past 3 months
  • You do not have severe vision or hearing problems, serious liver or kidney issues, or certain neurological or psychiatric conditions (like dementia, Parkinson's, or schizophrenia)
