Who can take part

  • You speak English or Spanish
  • You have high blood pressure and take at least one blood pressure medication
  • Your last two blood pressure readings at your doctor's office were 130/80 or higher
  • You've seen your primary care doctor in the last 6 months
  • You've had some trouble getting enough food (based on two screening questions)
