- You are 18 years old or older.
- You have had a heart attack, stroke, or artery surgery in the past.
- You have type 2 diabetes.
- In the last 3 months, your blood pressure was over 150 mmHg and your LDL (bad) cholesterol was over 130 mg/dL.
- You are not already a patient at Duke's special heart and diabetes clinic.
- You live in one of these North Carolina counties: Durham, Orange, Wake, Person, or Granville.
