Who can take part

  • You are 18 years old or older.
  • You have had a heart attack, stroke, or artery surgery in the past.
  • You have type 2 diabetes.
  • In the last 3 months, your blood pressure was over 150 mmHg and your LDL (bad) cholesterol was over 130 mg/dL.
  • You are not already a patient at Duke's special heart and diabetes clinic.
  • You live in one of these North Carolina counties: Durham, Orange, Wake, Person, or Granville.
