Steroid therapy with clot removal for large stroke
Treatments studied
Part of Brain & nervous system, Heart & circulation clinical trials.
This study tests if adding a steroid medication (methylprednisolone) to the standard procedure of removing a large blood clot from the brain can improve recovery for people with a severe ischemic stroke. It is for adults who can have the clot removal procedure and have a large area of brain damage.

Who can take part
- You are between 18 and 85 years old.
- You have had a large ischemic (blockage) stroke and can get treatment within 24 hours of when you were last seen well.
- Your stroke is severe enough that your NIHSS score is 6 or higher (doctors will check this).
- You have a blockage in a major brain artery (like the internal carotid or middle cerebral artery) and are scheduled for a procedure to remove it (endovascular thrombectomy).
- Your brain scan shows a large area of damage (ASPECTS 0-5 or core volume 70 mL or more).
- You can take steroids safely (no allergy, no active infection that would make steroids risky).
- You do not have any condition that would make the procedure risky, like recent bleeding, very high blood pressure, or kidney failure.

This trial tests if a procedure to remove a large blood clot from the brain (called a thrombectomy) can help people with severe strokes who have a large damaged area. It is for people who can be treated within 24 hours of their stroke starting.
This study tests if adding the steroid methylprednisolone to the standard clot-removal procedure (thrombectomy) can improve recovery for people with a large ischemic stroke, compared to thrombectomy alone.
This study tests whether a steroid called methylprednisolone, given soon after a flow diverter stent is placed, can help reduce inflammation and improve outcomes for people with an unruptured brain aneurysm. It is for patients who are already scheduled for this stent procedure.
This trial tests if giving a steroid (methylprednisolone) directly into the brain artery after successful clot removal improves recovery. It is for people who had a large stroke in the front part of the brain and got the clot out within 24 hours.
This trial tests whether injecting a steroid (methylprednisolone) directly into brain blood vessels after removing a stroke clot helps reduce brain swelling and improve recovery. You would be eligible if you had a large clot stroke in the front part of your brain that was successfully removed within the past 24 hours.
This trial tests if the steroid methylprednisolone helps people who have had a major stroke and also have low infection-fighting white blood cells (lymphocytes). It is for patients who are getting a procedure to remove the clot (thrombectomy) within 24 hours of the stroke.
