Focused Radiation for Large Brain Tumors (BIGSHOT)
Part of Brain & nervous system, Cancer clinical trials.
This trial tests a new way of giving focused radiation (SRS) to larger brain tumors (2 to 5 cm). It aims to safely treat these big tumors without whole-brain radiation, which can have more side effects.
Summary written for real people, not researchers, by Clin2.
Who can take part
- You are 18 years or older
- You have at least one brain tumor that is between 2 and 5 cm in size
- You have not had radiation to the whole brain or to the area of the large tumor before
- You are able to have an MRI with contrast dye
- You do not have cancer spread to the lining of the brain (leptomeningeal disease)

This Phase 1 study tests a form of focused radiation given in stages to shrink larger brain metastases. It may help people whose tumors are in the brain but not suited to whole-brain radiation alone, while also checking safety and how well the treatment can be delivered.
This Phase 2 study tests a precise, no-head-frame radiation technique to treat brain metastases. It may help by aiming radiation tightly at the tumors while limiting exposure to nearby healthy brain.
This trial tests a new approach for people with a single large brain tumor that has spread from another cancer. You would receive a short, focused dose of radiation (stereotactic radiosurgery) in small, separate treatments, followed by surgery to remove the tumor. The goal is to see if this combination works better than the usual treatment.
This study tests a more targeted form of stereotactic (precise) brain radiation for people with brain metastases from large cell cancer. It aims to improve the balance between effectiveness and side effects by carefully focusing where the radiation goes.
This Phase 2 trial tests a type of focused radiation (stereotactic radiosurgery, or SRS) for people with small cell lung cancer that has spread to the brain. It looks at whether this approach can treat brain metastases when the cancer has limited to a certain number of spots.
This Phase 1 study tests how to safely give focused radiation (SRS) at different doses to people with brain metastases. It aims to find a dose level that works well while limiting side effects.
