- You are 18 years or older
- You came to a participating emergency room and were diagnosed with a spontaneous (not caused by injury) brain bleed on your first CT or MRI scan
- The bleed was not caused by another condition like a brain tumor, abnormal blood vessels, a stroke, or recent head injury
- You are not under legal guardianship or conservatorship
