- You are between 30 and 79 years old.
- You have at least one metabolic risk factor, such as high blood pressure, high triglycerides, diabetes, or chronic kidney disease (stage 1–3).
- You get your primary care from Johns Hopkins Community Physicians.
- You have a working smartphone or tablet to use an app.
- You do not have a history of heart disease or stroke.
- You are not currently on or planning to start a GLP-1 medication (like Ozempic) in the next 6 months.
