- You were in the hospital for heart failure at a VA hospital or VA-paid community hospital within the last 3 months.
- Your heart's pumping ability is 40% or less (ejection fraction).
- You are not already on the best or full recommended heart failure medications at the right doses.
- You are receiving care at one of the 6 participating VA medical centers.
- You are not on comfort care only or hospice, and you are not currently unstable.
