Heart failure treatment comparison study
Part of Heart & circulation clinical trials.
This study compares three different combinations of medicines to see which one works best for people with heart failure and a preserved ejection fraction (a type of heart failure where the heart's pumping function is normal but it doesn't fill properly). Participants will receive one of three treatments: vicadrostat plus empagliflozin, empagliflozin alone, or empagliflozin plus spironolactone. The study looks at how these medicines affect proteins in the blood, which may help doctors understand which treatment is more effective.

Who can take part
- You must be 18 years or older.
- You have been diagnosed with chronic heart failure for at least 3 months and are in NYHA class II, III, or IV (meaning you have some symptoms like shortness of breath or fatigue).
- Your ejection fraction (a measure of how well your heart pumps) is greater than 40% (checked within the last year).
- You have a blood test result (NT-proBNP) that is above a certain level, depending on your weight and whether you have atrial fibrillation (an irregular heartbeat).
- You must have at least one of the following: are on a water pill (diuretic) or an SGLT2 inhibitor (like empagliflozin) for at least a week, were hospitalized for heart failure in the past 6 months, have elevated heart pressures, very high NT-proBNP, have protein in your urine (UACR >=30), or have certain structural changes in your heart seen on an ultrasound (echo).
- You are not taking certain other heart failure medicines (like spironolactone, eplerenone, or finerenone) within 14 days before starting the study.

This study tests an investigational medication called elecoglipron to see if it can improve heart and blood vessel outcomes in people with heart failure with preserved or mildly reduced ejection fraction. If you join, you will receive either the study drug or a placebo, and your heart health will be monitored over time.
This trial tests an investigational drug called VS-041 to see if it's safe and helps people with a type of heart failure where the heart pumps well but doesn't relax properly (HFpEF). It may be an option if you are 50 or older and have stable heart failure symptoms.
This study looks at people with a type of heart failure where the heart pumps normally but still doesn't fill properly. Researchers are using ultrasound pictures of the heart (echocardiograms) and blood tests to better understand this condition and find better ways to treat it.
This study looks for people who may have heart failure with preserved pumping function, where the heart is stiff and doesn't fill properly. It aims to better understand and identify this condition to improve care.
This trial studies people who have heart failure symptoms but whose heart pumping strength is largely preserved. It aims to better understand this specific type of heart failure and how best to treat or evaluate it.
This study tests whether adding a new study drug called vicadrostat to the standard heart failure medicine empagliflozin helps people with heart failure. It may be a good option if you have heart failure with preserved pumping function and are not already on a medicine called an MRA.
