Who can take part
- You have OCD for more than 5 years
- Your OCD symptoms are severe (Y-BOCS over 25 and/or key symptom score over 15)
- You’ve already tried multiple OCD treatments: at least 3 serotonin-boosting medication trials including clomipramine, plus add-on treatment (for over 4 weeks) with an antipsychotic and either lithium or clonazepam
- You’ve also done proper CBT with exposure therapy and response prevention (either you couldn’t tolerate it, or you had 15+ sessions)
- You can consent to the study and have medical clearance for surgery, anesthesia, and MRI
- You should not have certain situations (like hoarding-only OCD, poor insight, past bipolar/psychosis, recent substance issues, high current suicide-risk score, certain personality disorders, or significant brain/scanning problems)
