This study checks a blood test that looks for HPV tumor DNA to help doctors decide when to start routine treatment after surgery. It may help some people avoid starting treatment too early or too late based on how the test changes after the operation.

Who can take part
- You are 18 or older and can do daily activities at a good level (ECOG 0–2).
- Your cancer is HPV-16 squamous cell cancer in the oropharynx (throat) or HPV-16 head and neck cancer with no known starting point; HPV must be confirmed by a specific lab test (in-situ hybridization).
- Before surgery, your HPV blood test (ctDNA) must be detectable at least 50 copies per mL using the Naveris digital PCR test.
- You must have had surgery to remove all visible cancer, and scans after surgery should show no visible disease.
- After surgery, you must have two blood tests (about 2–6 weeks after surgery) that are both negative (less than 1 copy per mL).
- Your pathology (surgical tissue results) must match certain higher-risk features (Arm A or Arm B), such as specific lymph node spread patterns, lymph/nerve invasion, or margin concerns.
