SGLT2 medicine for high-risk people after sudden worsening heart failure

This Phase 3 trial tests whether starting an SGLT2 inhibitor early in the hospital can improve outcomes for people at high risk after acute (sudden) heart failure. If you qualify, you would be randomized to receive the study medicine or standard care, started within 12 hours of admission.

Who can take part

  • Be at least 20 years old and consent to take part (or have a Mini-Cog check if age 90+)
  • Be hospitalized with sudden heart failure that needs IV loop diuretic “fluid medicine”
  • Have shortness of breath at rest or with slight activity, plus at least 2 signs of fluid buildup (or meeting BNP/NT-proBNP blood test thresholds)
  • Have evidence of higher risk, such as weaker kidney function, higher-dose home furosemide use, or low urine output right after IV furosemide
  • Not have been on an SGLT2 inhibitor in the last 3 months
  • Not have conditions that make SGLT2 use unsafe, such as very low blood pressure, risk of severe blood sugar complications, or planned dialysis
