This study tests whether a lower dose of alteplase (a clot-busting medicine) given into the chest can improve healing of infected pleural fluid when drainage is incomplete. It may help reduce dosing-related side effects while still helping clear the infection-related fluid.

Who can take part

  • You currently have an infected pocket of fluid around the lung (pleural infection) and the chest drain has not fully drained it after at least 1 day
  • You already have a chest drain in place (French size 12 or larger) and you’ve started antibiotics
  • Your care team plans to use medicine through the drain that helps break up clots (intrapleural fibrinolytic)
  • You are able and willing to sign informed consent
