- Men over 55 or women over 65
- Type 2 diabetes, prediabetes, or metabolic syndrome (a condition with at least 3 of: large waist, high triglycerides, low good cholesterol, high blood pressure, or high blood sugar)

- No history of heart attack, stroke, angina, or other significant heart or blood vessel problems
- Must be able to safely have a special heart CT scan (no severe kidney disease, allergy to contrast dye, or weight over 300 lbs)
- Must be willing to follow the study plan and have a life expectancy of at least 3 years
